AGADA Physical Therapy & Integrative Health Center is often the first conservative choice for women suffering with incontinence and pelvic pain, avoiding a treatment course that includes medication or surgery. Our specialized physical therapist has advanced training in the anatomy and the role of muscles and structures in and around the pelvis. These muscles work to support the bowel and bladder, stabilize posture during activities and assist in sexual activity. Problems with these muscles can result in pain, bowel and bladder difficulties, and can interfere with intimate relations.

What to expect with AGADA PT’s Incontinence and Pelvic Pain services:

AGADA’s treatment is specialized, highly professional and private. The therapist will provide a thorough evaluation, including a complete history of the problem, postural assessment, and muscle testing. In some cases a vaginal examination of the pelvic floor muscles will also be performed. A customized treatment program will be developed that includes:

Muscle re-education for postural correction, relaxation, and/or strength training.

Manual therapy for correction of joint mechanics and soft tissue restrictions.

Self management home programs that include exercise, body mechanics, education in diet and other behavior strategies to reduce and manage. Symptoms.

Our goal is to help our patients self-manage their condition. Individual cases vary in the amount of time needed to meet specific functional goals. An average range of visits for incontinence is 6-8 visits, for pregnancy and post-delivery 6-8 visits, and for pelvic pain 7-9 visits.

Dry Needling

Dry Needling

Dry Needling is a general term for a therapeutic treatment procedure that involves multiple advances of a filament needle into the muscle in the area of the body, which produces pain and typically contains a ‘Trigger Point’. There is no injectable solution and typically the needle that is used is very thin. Most patients will not even feel the needle penetrate the skin, but once it has and is advanced into the muscle, the discomfort can vary. Usually a healthy muscle feels very little discomfort upon insertion of the needle; however, if the muscle is sensitive and shortened or has active trigger points within it, the subject may feel a sensation much like a muscle cramp — which is often referred to as a ‘twitch response’.

The twitch response also has a biochemical characteristic to it which likely affects the reaction of the muscle, symptoms and response of the tissue. The patient may only feel the cramping sensation locally or they may feel a referral of pain and/or similar symptoms for which they are seeking treatment.

A reproduction of their pain can be a helpful diagnostic indicator of the cause of the patient’s symptoms. Patients soon learn to recognize and even welcome this sensation, as it results in deactivating the trigger point, reducing pain and restoring normal length and function of the involved muscle.

Typically, positive results are apparent within 2-4 treatment sessions but can vary depending on the cause and duration of the symptoms, and overall health of the patient. Dry needling is an effective treatment for acute and chronic pain, rehabilitation from injury, and even pain and injury prevention, with very few side effects. This technique is unequaled in finding and eliminating neuromuscular dysfunction that leads to pain and functional deficits.

Injury Risk Assessment

AGADA Physical Therapy & Integrative Health Center uses software developed by Move2Perform that generates a risk assessment report. Based on the findings of each of these tools.  If an athlete falls under the “moderate deficit or substantial deficit” categories, physical therapy and corrective exercises are highly recommended.

Several programs have been developed to help decrease an athlete’s risk of injury. UC-Santa Monica developed the PEP Program. Prevent Injury, Enhance Performance. Research performed by Gilchrist et al. in 2009 confirmed excellent outcomes with the use of the PEP program including overall incidence of ACL injury was 1.7x less likely, and non-contact injuries were 3.3x less likely.

ACL Injury Prevention

Chances are if you play sports, or have a child in sports, you know someone who has torn their anterior cruciate ligament (ACL). Females have a 2-8x higher risk of ACL tear based on intrinsic and extrinsic factors. Intrinsic factors include hormonal and anatomic differences including wider pelvis, increased Q-angle, knee valgus, and foot pronation.

Extrinsic factors include Biomechanical and Neuromuscular considerations such as: cutting and landing with more erect hip/trunk and less knee flexion; “quadriceps dominant” (males fire their hamstrings 3x the level of females when landing from a jump and cutting), and muscle fatigue/ dynamic stability. The cost for surgical repair of a torn ACL is an astounding $17,000. The overall cost is $680 million/ year.

Coaches, Parents, Athletes should be asking 2 very important questions. How can we determine who is at risk for injury? What can we do to decrease that risk and prevent injury?

Who couldn’t use a little extra push to get more exercise!? Swen received her Certified Personal Training certificate through NASM in 2006. She is currently studying for the Certified Strength and Conditioning Specialist (CSCS) certification exam. Swen has more than 6-years of experience with fitness training. Swen utilizes circuit training and HIIT training in order to maximize results for her clients. Home fitness visits are available within a 4-mile radius of AGADA Physical Therapy.

Walk With Me

Did you know that if you exercise with a “buddy” research shows that people workout harder and longer? Sometimes you just need a little motivation to get out the door, and that’s what this service provides. If you live within a 4-mile radius of Agada Physical Therapy & Integrative Health Center, your therapist will come meet you for a 30-minute walk around the neighborhood or on the trails. Your therapist will bring a heart rate monitor and pedometer to monitor your progress and calories burned. We can also discuss the use of resistance bands or small hand weights for some upper body work as we walk.

For most healthy adults, the Department of Health and Human Services recommends at least two hours and 30 minutes a week of moderate aerobic activity or one hour and 15 minutes a week of vigorous aerobic activity — preferably spread throughout the week — and strength training exercises at least twice a week.
