Insurance Coverage

Thank you for your continued support and patronage of AGADA Physical Therapy and Integrative Health Center!  Beginning January 1, 2025, AGADA PT will be transitioning to a cash practice.  I will provide a discount for seniors and medical assistance recipients with a signed ABN on file (provided at the clinic).  As with the other providers at AGADA IHC, I will provide a receipt or “superbill” that you may submit to your insurance provider.  I truly appreciate your support during this transition!
